The pathway encompasses Living Well Values and provides a structured approach to ensure people’s holistic needs are met quickly and efficiently. The initial conversations template is
used when a person is first introduced to the new model. The citizen and a staff member complete it together. It normally
takes 20 minutes to complete.

The Living Well plan is a chance to decide what story to share with the different people in your life. The cards ask you about your passion, strengths, hopes and what support will work best for you.
You can choose the parts you would like to fill in.

Wellbeing cards give you different ideas and tips about how you can improve your wellbeing, and manage your emotions, in different situations.

The well-being pack also contains details of organisations that you may want to contact in the future.

These questions can help people to reflect on their lives and take action to improve their well being. The difficulties people face are often subtle and it isn’t easy to identify goals and actions. Coaching helps people to explore their situations and make positive changes.
